Question
Will the Minister outline what plans are in place to reduce bed blocking in our hospitals?
Answer
Answered by Minister for Health and Social Services
Renewed and revised expert guidance to assist staff in safe and timely discharge of hospital patients will be launched on 6 December.
In addition, a new system to record discharge delays, “Pathways of Care Reporting” is to be implemented that incorporates a range of improvements and new measures to form a standardised system that provides comparable data.
The new framework is being implemented in a phased approach. The first phase was piloted by three health boards and provided valuable testing that enabled further improvements and requirements to be made to the system. Phase two commenced recently as an all-Wales pilot to allow further testing and development and will produce a full data set and overview across Wales. This phase will run until the end of January 2023 when a review of the outcomes will be completed before the live system commences.
