What is the Welsh Government doing to combat chronic conditions?
Ateb
Atebwyd gan Cabinet Secretary for Health and Social Care
A Healthier Wales is our long-term strategy which focuses on everyone in Wales having longer healthier and happier lives, with an emphasis on wellbeing, prevention and early intervention, whilst supporting high quality services as close to home as possible. We are currently refreshing the actions for this strategy which will continue to focus on supporting people to manage their own health and wellbeing and for health and care services to be more proactive in preventing or delaying the onset of long-term health conditions.
As a part of this refresh, there will be renewed actions by Public Health Wales to support people to make healthy choices about their lifestyles to reduce their risk of avoidable long term health conditions.
There will also be a much stronger drive to implement the Primary Care Model for Wales. Central to this model is multi professional and multi-agency teams working in communities to deliver more proactive planned care and support to help people to avoid or delay the onset of preventable long term health conditions.
To inform the planning and delivery of high quality safe and sustainable services for long term health conditions, there is a suite of nationally agreed Health Pathways and a series of quality statements. Much of the focus of the quality statements is on improving chronic condition management to support better outcomes, reduced rates of complication, and more sustainable healthcare services.
