How many unexpected deaths were there following hospital discharge in each of the past three years for each local health board?

Atebwyd gan Cabinet Secretary for Health and Social Care
This information is not collected by Welsh Government or the ONS. All deaths are reported to the All-Wales Medical Examiner Service but it does not currently publish statistical reports. The service independently scrutinises all deaths that are not investigated by the coroner. The Medical Examiner will ensure that an accurate cause of death is recorded and identify any which are subject to a mortality review by the health board. Health boards should have a list of all deaths and can interrogate how many were post discharge.
